Why isn't South Africa included in the countries that you can call in the £5 and £10 Add ons?

If I start a list would people like to add to it?

1. Insufficient demand
2. Local carriers want too much money
3. It did not occur to those who negotiate these deals to include SA
4. Technical/legal issues
5. ...
